Requesting Medical Records

To respect the privacy of our patients and to comply with the federal Health Insurance Portability and Accountability Act of 1996 (HIPAA), the following requirements must be met before the release of medical records.

Palmyra Ambulance Association, Inc. (Palmyra-Cinnaminson-Rverton EMS) requires a subpoena and/or witnessed medical release form signed by the patient.

A processing fee of $25.00 payable by check, made out to Palmyra Ambulance Association, Inc.

Release of medical records will not occur until both of these requirements are met and mailed to:
